Surgery through the laparoscopic approach ("keyholes operations") has been a breakthrough in general surgery for the past 20 years. It seems obvious that bariatric surgery has greatly benefited from this approach. Nowadays, almost all the current bariatric operations are performed this way.Yet this is still surgery, with a general and deep anaesthesia, an hospitalization, and post-operative risks, although this can be done also on an out-patient basis. Can we do better, i.e.operate through natural orifices

Only two methods are available for the time being: the intragastric balloon, that we shall here once more address, and endostapling techniques.
Intragastric balloon:as our readers know, the balloon is the most ancient non invasive technique, and has provided fair results over the time provided once its limitations are understood !

- Procedures with stapling : Whereas the gastric balloon is widely used, endoscopic stapling is a confidential and experimental method. A study has been launched in the USA (FDA), while pilot centers are operating in Mexico and Belgium. Preliminary results are acceptable, but should be improved...

Intragastric Stimulation
Is there a future for gastric electrical stimulation for obesity treatment?
Some years ago (2000-2004), a promising large scale trial took place in Europe and in the US. A gastric pacemaker (Intra-gastric Stimulation or IGS), which is a simple, mini-invasive and elegant operation, was proposed to obese patients. Please refer to the chapters ('digestive surgery') dealing with this operation, technical details and initial results.
Robotic system and bariatric surgery
Robotic is a recent advance in surgery, particularly in video-assisted mini-invasive surgical procedures requiring manual sutures. Indeed using stitches and manipulating sewing intruments can be greatly facilitated by a robotic system. One of the first promising fields has been heart surgery (coronary bypass). Different procedures have been attempted in laparoscopic abdominal surgery with a robotic system, and recently gastro-jejunal bypass.
