Eating questionnaire

The eating questionnaire is a tool that will help the professional who deals with nutrition to point out the food habits of a patient, thus determine his level of calorie-intake, and evaluate the outcomes, both in quality and quantity. In order to obtain these data, several talks are required.

The various topics that will be addressed are:

Meals: rhythm, frequency, time allocated to each meal, place and environment, guests, size of the meal, collations, nightly snacks...

History of the patient: age, family and medical background, diets already followed, circumstances of weight-gain…

- Life habits : professional life and its necessities, social life (invitations…), physical activities.
- Taste preferences : salty or sweet foods, association of salty and fat foods, or sweet and fatty foods.
- Budget limits.

The questionnaire will also look at what food is ordinarily consumed, and the way it is prepared. Food intake will thus be analysed bases on the following:

- main nutrients: proteins, lipids, glucides
- minerals and vitamins
- fatty acids
- fibres…

This will make it possible to calculate the total energy intake, as well as the food-balance and to detect possible nutritional deficiencies.

The clinical elements are also relevant: height, weight, BMI, measurement of the hip and waist, and waist-hip ratio…

The food questionnaire is an essential tool, that will enable the physician and the dietician to propose an adapted outline for food intake, combining weight-loss and nutritional balance.
